Quest Hair Collections

This Tip Sheet will demonstrate how to perform a hair collection with Quest Laboratory in the eScreen123™ system. 

For information on checking in a donor, please reference the Quest Collections Tip Sheet located in the Tools section of eScreen123.com.  

STEP 1: VERIFY INFORMATION

1. Verify the donor’s information based on their valid photo identification.
2. Click Next Step to proceed. 

NOTE: Access the Quest Quick Start Guide for detailed instructions on the Quest hair collection process.

STEP 2: COLLECT SPECIMEN

1. Prepare for the collection by locating the Quest hair collection kit and sanitizing the scissors in the presence of the donor.
2. Indicate if the hair collection will be from the head or body. If you select body, you much specify the location. You cannot mix head and body hair for the collection.
3. Review the collection steps and click the check boxes by each step as you complete them.

4. Click Next Step, once all the boxes have been checked.

STEP 3: GENERATE eCCF

1. Indicate the shipper.
2. Add any notes that you would like to appear on the chain of custody.
3. Scan the seal on the barcode, verify it for accuracy and click Validate CCF

4. Click Generate to create an eCCF to match the specimen seal.

 

SIGN ELECTRONICALLY

 

1. Click Sign with ePad.

 

2. Advise the donor to read their statement and sign electronically. Click Commit Signature to save the donor’s signature and proceed to the collector’s signature.

3. Read the collector statement and sign electronically. Click Commit Signature to save the collector’s signature and proceed.

4. Print the CCF and give a copy to the donor.

SEND TO LAB

1. Indicate the Shipper.
2. Click Test Complete to finalize the test.
