IDENTIFYING AND CHECKING IN QUEST COLLECTIONS

Quest Diagnostics collections are easy to identify. Look for Quest Diagnostics as the laboratory performing
testing on the authorization form or the chain of custody. All Quest Diagnostics authorization and chain of
custody forms will follow the check check-in protocol used for your eScreen National Account customers.

Easily identify Quest Diagnostics Collections by looking for their name on the authorization form or chain of custody.

BEGINNING THE COLLECTION IN eSCREEN123 

Our check-in process is designed to consolidate multiple check-in steps and help you use the information
presented to you. Your check-in method will be defined by the donor’s authorization or chain of custody
form. For your convenience, we are providing the check-in process details for each form.


Check-In Using Authorization Form 

Quest Diagnostics authorization forms are used the same way an eScreen ePassport is used to start
a collection in eScreen123.

1. From the Donor Waiting List, ensure that ePassport or PeoplePass is selected in the dropdown.
2. Scan the QPassport ID (may also be labeled Reference Test ID, eReq #, or Authorization Barcode #) into the Scan or enter barcode field.

Next Steps Quest Authorization Form

1. If you would like to immediately start the event, click Start Event.
2. If you would like to start the event later, click Move Donor/Patient to Waiting List.

Check-In Using Chain of Custody Form 

The lab account number on the chain of custody form can be used to locate the account in eScreen123.

1. From the Donor waiting list, select Chain of Custody Form in the dropdown.
2. Select Quest in the second dropdown.
3. Enter the lab account number into the Lab Account # field. Click search

Click on the employer’s name to begin the collection.

You will manually enter all the donor’s information with this check-in method. 

Preparing Specimens for Shipment 

Specimens collected by your clinic for Quest Diagnostics must be sent to the correct laboratory to prevent
delayed reporting to the customer. Always review the lab information notated in the upper right-hand
corner of the eCCF before shipping.

Shipping Specimens to the Laboratory 

Quest Diagnostics utilizes couriers and FedEx to deliver specimens to their laboratories. Your clinic will
have a designated shipping method. Depending on shipping density, couriers will be scheduled daily or on
a will-call basis. 

READY TO SHIP? 

Courier - Contact Quest Diagnostics at 866-697-8378. You will be asked to provide your logistics account
number or address. 

FedEx – Contact FedEx at 800-463-3339.

MRO Copy of Chain of Custody for DOT Collections 

Collectors are required to send the MRO copy of the chain of custody form directly to the MRO. MRO contact
information can be found on the chain of custody form. The MRO copy of the DOT eCCF generated in
eScreen123 will be faxed to the MRO by eScreen on your behalf. But collectors must also send the MRO
copy for any collection performed using a paper chain of custody form to the MRO.

Supply Orders 

Quest Diagnostics collection supplies can be ordered in eScreen123 through the Place Order tool.
FedEx Waybills can also be ordered by those clinics not assigned to a courier.

Collection Reimbursement 

Your clinic will be reimbursed for collections performed for Quest Diagnostics according to the rates
and payment terms in your existing eScreen service agreement.
