New (2020) Federal CCF Required Effective August 2021

Guidance for using the 2020 Federal custody and control form (CCF)

The Office of Management and Budget (OMB) has approved the 2020 Federal Custody and Control form for use. The prior version of the Federal custody and control form expires on August 29, 2021.

Important Information

Where available, the use of a Federal electronic CCF (Federal eCCF) solution will help eliminate expired form delays. It provides timely routing of CCF copies to appropriate stakeholders, reduces errors of omission, and ensures appropriate fields are completed by specimen type.

Beginning August 30, 2021, the use of an expired Federal CCF will require corrective action. This could cause extensive delays in result reporting or potential rejection of specimens if not addressed.
